Before Your Procedure
Please read, complete, and sign all requested forms and bring with you on the day of your procedure.
Make sure to bring a photo ID, your insurance card, and any required payment due at the time of services to the facility. Remember to leave all valuables at home.
Be sure to follow your physician’s instructions on preparing for your procedure. A nurse from the facility will also be calling within a couple days of your procedure with additional steps to follow. Any instructions not followed could cause a delay or cancellation of your procedure.
